Public Health Services at SCHSS offer a wide range of supports to promote community health and wellness. Services include medical management, wound care, nursing support, and the Aboriginal Diabetes Initiative, which focuses on diabetes education and prevention. The Home and Community Care program assists Elders and those with chronic conditions, while the Medical Community Van provides transportation to appointments. The Stoyoma Dental Clinic serves over 7,000 clients from across the region and offers free patient transport on Tuesdays and Thursdays. The clinic also supports schools through the Children’s Oral Health Initiative, and distributes dental supplies to local health units and surrounding communities. SCHSS also provides water testing services to help ensure safe, healthy living conditions.

Community Health

Immunization Program

Our immunization program helps protect community members against vaccine-preventable diseases. We provide vaccinations for all ages, from infants to Elders, following provincial guidelines. Services include routine immunizations, seasonal flu shots, and catch-up schedules, with education on vaccine safety and effectiveness.

Water Testing Program

In partnership with Scw’exmx Water Technicians, SCHSS conducts weekly water sampling from all well systems in the Coldwater and Nooaitch communities to meet First Nations Health Authority (FNHA) standards. For homes experiencing ongoing water issues, bottled water deliveries ensure every household has safe, clean drinking water.

Prenatal, Postpartum & Infant Program

Our Prenatal, Postpartum & Infant Program offers comprehensive support for expectant parents, new parents, and families with infants. Community Health Nurses provide prenatal education, postpartum follow-up, infant growth monitoring, and guidance on healthy early childhood development. We work closely with families to ensure a safe, healthy start for every child, while connecting them with additional community resources when needed.

Car Seat Loaning Program

We offer a short-term car seat loan program to help families ensure child passenger safety for travel to appointments, community events, and personal needs.

Canadian Prenatal Nutrition Program (CPNP)

The CPNP supports healthy pregnancies through nutrition education, supplemental food resources, and referrals to other community programs. Our team works with expectant parents to encourage healthy eating habits for both mother and baby.

Breast Pump Prescription Support

Breastfeeding parents can access breast pump prescription support to help maintain milk supply, particularly for newborns in hospital care or for parents returning to work or school.

Parents & Tots Programs (Infant/Toddler Group)

These group sessions create a welcoming space for parents and caregivers to connect, share experiences, and access early childhood resources, while toddlers enjoy play-based learning.

Food Preservation & Security

We offer canning workshops, nutrition programs, and food-safe certification courses to support food security and healthy eating. Community members can learn safe preservation methods and gain skills for long-term food storage.

Hearing Clinics – Annually

Annual hearing clinics provide hearing assessments, education, and referrals to specialists, ensuring early detection and management of hearing loss.

Orthotic Clinics – Annually

Our yearly orthotic clinics offer assessments and fittings for custom orthotics, helping improve mobility and reduce discomfort from foot, leg, or back issues.

Swim & Gym Program

The Swim & Gym program encourages physical activity through group exercise and swimming sessions, promoting fitness and community connection.

Cervical Self-Screening

We provide resources and support for cervical self-screening, giving individuals the tools to take a proactive role in their reproductive health.

Home & Community Care Health

Home Care Nursing

Registered Nurses deliver in-home care for Elders, individuals with chronic illnesses, and those recovering from illness or surgery. Services are culturally sensitive and designed to support independence while meeting health needs.

Foot Care Services

Specialized foot care is available for community members, including nail care, callus removal, and education on foot health—especially important for those living with diabetes or circulation issues.

Frozen Meal Program – Dinners at Home

Nutritious frozen meals are prepared and delivered to clients who may have difficulty cooking, helping maintain a balanced diet.

Medic Aid Response System

We support the use of personal medical alert devices to ensure clients can quickly access emergency help when needed.

Aboriginal Diabetes Initiative

This program reduces the risk and impact of Type 2 diabetes through culturally relevant education, prevention, and health promotion. Services include blood testing, eye exams, diabetic nurse educator visits, orthopedic and mastectomy clinics, food skills courses, and monthly walking/vitals group sessions.

Nutrition Counselling

Clients can meet with health professionals for personalized nutrition guidance to support chronic disease management, weight control, and overall wellness.

Case Management

Our team coordinates health services, referrals, and resources for clients with complex needs, ensuring they receive comprehensive, ongoing support.

Wound Care Services

Skilled nurses provide assessment, treatment, and monitoring for acute and chronic wounds, promoting healing, preventing infection, and educating clients on self-care techniques.

Medication Administration

We assist with safe and accurate medication administration, including reminders, monitoring, and coordination with prescribing physicians.

Chronic Disease Self-Management

We empower clients to better manage chronic conditions through education, goal-setting, and regular follow-up care, helping reduce complications and improve quality of life.
